Located in Shuster Hall, Rm. 106. Telephone: 718-960-8652. The Records and Transcript area maintains and processes all information pertaining to a student's academic record. This includes personal information, course registrations, grades, major/minor, transfer credit and credit taken on permit.
DECLARATION OF PLANS AND/OR SUBPLANS
(Majors, Minors, Advanced Certificates, Tracks, Concentrations, and Specializations)
Students must declare a plan by the time they have earned/ transferred 60 credits. Students can obtain the declaration of plan/sub-plan form in Shuster Hall 106 or in the online forms section in the Office of the Registrar website. For students pursuing their second bachelor's degree, the Second Bachelor Degree-Area of Concentration form is required. The form can be found in the Office of Academic Standards & Evaluation in Shuster Hall 280.The completed form must have all required signatures for processing.
This form must be submitted by the term deadline (Census date) which is stated in our academic calendar; forms submitted after the Census date will be processed for the following term.
CHANGE OF NAME, NATIONAL ID AND ADDRESS
Students can visit Shuster Hall 106 to initiate a change of name. A change of name form must be submitted in person accompanied by appropriate original legal documentation (e.g. marriage license or annulment/divorce papers, court order, birth certificate and/ or Certificate of Naturalization). Photocopies will not be accepted.
Change/correction of National ID must also be done in person accompanied by an original, signed Social Security Card or Taxpayer Identification Number (if student does not have a Social Security number).
For address changes, students may submit their request in person as well as in Self Service in CUNYfirst*.
First semester students should report to the Admissions Office, Shuster Hall 161 to change their address.
*Students will only have access to change their billing, mailing and home address types in Self Service. To change their permanent address, they must submit the request in Shuster Hall 106.
These forms can be returned to Shuster Hall 112 during evening hours when classes are in session.
