Documents Required for Registration

• High School Transcript or S.A.T. Report (Proof of Academic Eligibility: 75+ on E.L.A.; regents exam or 480+ on S.A.T./Verbal/Critical Reading).
  
• Medical Requirement Waiver signed and dated by parent/guardian (Students do not need to see a physician.)
  
• Social Security Number
