  • Adjunct Form
  • To provide data for all appointed adjuncts each semester the Department Chair completes and submits to the Dean's office the Adjunct Form.

  • Chair Annual Self-evaluation Form
  • To provide context for the Chair's annual academic year-end Dean’s evaluation conference, prior to it and along with the Department Annual Report Form, the Department Chair completes and submits to the Dean the Chair Annual Self-Evaluation Form. Following the conference, the Chair will receive the Dean’s summary, comments, and agreed upon goals for the following academic year. Then each signs it, and Chair retains original, while Dean retains copy.

  • College Assistant Timesheet Form
  • To provide biweekly cumulative workload report information for College Assistants, and according to scheduled deadlines, the CA completes and signs, then obtains their supervisoring department chair's signature, and submits to Human Resources  the College Assistant Timesheet Form.

  • Computer Lab Request Form
  • To request a computer lab in the Academic Computer Center for one or more of their existing courses, according to room scheduling's schedules, the Faculty Member completes and submits to the Registrar's office room scheduler the Computer Lab Request Form.

  • Curriculum Vitae CUNY/Lehman Template
  • To provide information required for all personnel actions including tenure and promotion, prior to their annual evaluation, and along with the Faculty Annual Evaluation Form, the Faculty Member updates and submits to the Department Chair the College Curriculum Vitae CUNY Lehman Template.

  • Department Annual Report Form
  • To provide context for the Chair's annual academic year-end Dean’s evaluation conference, prior to it and along with the Chair Annual Self-Evaluation Form, the Department Chair completes and submits the to the Dean the Department Annual Report Form.

  • Faculty Annual Evaluation Form
  • To gather information for each annual faculty evaluation the Department Chair or designee completes the Faculty Annual Evaluation Form. Then it is signed by the Faculty Member, who provides comments to the Department Chair, and then by the Chair. The original is to be filed in department, and a copy is provided to the Faculty member.

  • Faculty Report on Suspected and/or Adjudicated Academic Dishonesty Form
  • To provide information of any instance of student academic dishonesty that arises, the Faculty Member completes and submits to the Department Chair; to the Academic Standards and Evaluations Director; and to the Dean of Students in the Office of the Vice President of Student Affairs, the Faculty Report on Suspected and/or Adjudicated Academic Dishonesty Form. Faculty keeps the original report and all support materials.

  • HEO Series Evaluation Memorandum Form
  • To provide written evaluation of each HEO series employee's performance one to two times annually, the HEO's supervisor (designated by the President or Dean) conferences with, then completes, signs, obtains the HEO's signature, then submits to the Dean, the HEO Series Evaluation Memorandum Form.

  • Lecture Hall Request Form
  • Form to be submitted to the Registrar's office to request assignment of an existing course to a lecture hall; the form must be signed and dated prior to submission to the Registrar.

  • Lehman Stages Space Request Form and Contract
  • To request and gain approval to use of any of the Lehman Stages venues, including the Lovinger Theater, the Studio Theater, the Dance Lab, or the Student Experimental Theater/SET, complete and obtain Department Chair, Dean, and Lehman Stages Director signatures, then submit to Lehman Stages the Lehman Stages Space Request Form and Contract.

  • New Tenure Track Faculty Reassigned Time Research Plan Form
  • To define and schedule a new tenure track Faculty Member's five-year research agenda, during their first semester, the new tenure track Faculty Member must complete and submit, first to their Department Chair who signs it, and then to the Dean, the New Tenure Track Faculty Reassigned Time Research Plan Form.

  • Non Teaching Instructional Staff Timesheet [PDF]
  • Online Teaching and Learning Request for Proposals - RFP [PDF]
  • Forms to be completed by faculty interested in submitting a proposal to participate in the Online Teaching and Learning workshop series offered in the School of Arts and Humanities; forms must be signed by the applicant and Department chair and submitted to the dean's office by the due date specified in the RFP.

  • Summer 4-Day Work Week Schedules Form

To delineate instructional and non-instructional staff, summer work schedules by mid-May the Department Chair completes, signs and submits to HR the Summer 4-Day Work Week Schedules Form. Use one form for each of the two different work groups.
